Conducting comprehensive psychosocial assessments of patients and their families to identify their needs, strengths, and resources
- Developing and implementing care plans that address the psychosocial needs of patients and their families
- Providing counseling and emotional support to patients and their families, including grief and loss counseling
- Coordinating community resources and services to meet the needs of patients and their families
- Serving as an advocate for patients and their families, helping them to navigate the healthcare system and access needed resources
- Participating in interdisciplinary team meetings to develop and review patient care plans
- Maintaining accurate and timely documentation of all services provided
- Master's degree in Social Work from an accredited school of social work
- Current state licensure as a Licensed Clinical Social Worker (LCSW) or Licensed Master Social Worker (LMSW)
- Minimum of 2 years of experience working in hospice or palliative care
- Excellent communication and interpersonal skills
- Ability to work effectively as part of an interdisciplinary team
- Knowledge of community resources and services that can support the needs of hospice patients and their families
- Ability to provide compassionate and empathetic care to patients and their families
