Our Rapid Re-Housing program helps people exit homelessness quickly by connecting them to permanent housing and providing short-term financial assistance and supportive services. We meet clients where they are, supporting housing search, move-in costs, and ongoing stabilization so clients can build self-sufficiency and long-term success.
The RoleAs a Case Manager I, you will provide hands-on support to individuals, families, and transitional-aged youth (TAY) as they move into stable housing. You will work closely with clients to create personalized plans, connect them to essential resources, and provide guidance and advocacy that empowers long-term success. This is a role for someone who is compassionate, client-centered, and energized by helping people achieve meaningful change.
YourRole in Helping People Secure Housing
- Manage a caseload of up to 23 households, providing direct services to culturally diverse clients.
- Conduct regular meetings and monthly home visits to assess needs, monitor progress, and support housing stability.
- Develop and maintain solution-focused case plans that promote self-sufficiency and housing retention.
- Connect clients to internal and community resources, including rental assistance and supportive services.
- Advocate for clients with landlords, property management, and service providers to resolve housing-related concerns.
- Travel throughout San Diego County to support client engagement and access to services.
- Participate in multidisciplinary team meetings and maintain accurate, timely documentation in CSTAR in compliance with HIPAA and program requirements.
- Follow HIPAA standards, program policies, and documentation requirements.
Minimum Requirements
- Bachelor’s degree in Social Work, Human Services, Psychology, or a related field, or equivalent combination of education, certification, and relevant experience.
- Two years of full-time experience demonstrating reliability, adherence to guidelines, teamwork, and providing professional support to clients or community members, preferably in case management, social services, or similar environment.
- A valid driver’s license is preferred to support occasional travel between locations.
- Experience supporting individuals with disabilities, mental health conditions, or other challenges.
- Knowledge of strengths-based or solution-focused case management approaches.
- Strong problem-solving, organizational, and communication skills.
- Ability to manage multiple priorities in a fast-paced environment.
- Professionalism, discretion, and respect when handling confidential information.
- Knowledge of HIPAA standards and experience with compliance.
- Comfortable working in office, home, and community settings.
- Proficiency in typing and Microsoft Office software.
- Strong data entry skills with a high degree of accuracy.
This is a full-time position, typically 40 hours per week, scheduled Monday through Friday. Work hours may vary slightly based on client needs, housing appointments, or program requirements.
Compensation & BenefitsPay is based on experience, qualifications, and internal equity. The anticipated starting pay range for this role is $21.82 to $24.56 per hour.
