Cheer Home Care is looking for an RN Care Case Manager who loves spending time with older adults and provides high quality care in home. The RN Care Case Manager collaborates with interdisciplinary team members to develop and implement individualized care plans and provides feedback to clients, client representatives, caregivers and nurses.

They work closely with our office team and have excellent communication and problem-solving skills.

• Serve as the primary care manager, coordinating care to the clients among caregivers, RNs, therapists and outside representatives.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans and provide feedback to clients, client representatives, caregivers and nurses.
• Work closely with the office team and maintain excellent communication and problem-solving skills.
• Option to work one weekend day and have one weekday off.

• Upbeat Personality
• Active California RN License, required
• Master in Nursing, preferred
• Industry Experience, preferred
• Strong Computer Skills
• Ability to multitask, drive, take action
• Strong Organizational & Communication Skills
• Pass Live Scan Fingerprint Clearance
• Valid driver s license and proof of insurance required
• Cell phone with Internet Access
• Proof of Negative TB results
• Willing to Work One Weekend a Month as Back-Up On Call

Salary $120,000+ based on experience. Potential for team bonus.
