Registered Nurse (RN) – Home of Guiding Hands

Join our team to make a difference in the lives of individuals with intellectual and developmental disabilities. You will deliver person‑centered care across our homes and programs, collaborating with families, providers, and interdisciplinary team members.

Pay and Benefits

Base pay range: $85,000.00/yr – $/yr

Sign‑on bonus: $5,000

Key Responsibilities What You'll Be Doing Nursing Process & Clinical Assessment
  • Utilize the nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of clients’ care, as evidenced by client/staff observation.
  • Assess clients’ health care needs; collect and document current health status.
  • Make accurate and ongoing assessments of clients’ status and respond appropriately.
  • Evaluate and analyze clinical situations and make decisions within the scope of licensed practice.
  • Perform evaluation of new and existing clients to determine treatment priorities and need for modifications.
  • Demonstrate necessary skills and knowledge to provide care tailored to each client’s needs.
  • Handle emergency situations promptly, precisely, and professionally.
Care Planning & Implementation
  • Set priorities for nursing action in a logical sequence based on clients’ needs and medical providers’ orders.
  • Formulate individualized plans of care.
  • Perform ongoing assessment and revise plans of care based on new data or changes in clients’ conditions.
  • Ensure all clinical activities are carried out in accordance with each client’s Health Care Plan.
  • Provide personal client care to promote comfort, dignity, and well‑being while acknowledging physiological and psychological needs.
  • Observe and document clients’ responses to medications.
Documentation & Record Management
  • Document accurate, legible, and timely assessments of client status.
  • Record nursing interventions, client responses, needs, problems, capabilities, limitations, and progress toward goals.
  • Include documentation of all relevant client/staff teaching and demonstrated understanding.
  • Maintain up‑to‑date information on appointments, procedures, and health‑related updates in each client’s chart.
  • Compile data for periodic assessment of physical condition and evaluation of health care effectiveness.
  • Document all communication with physicians, families, case managers, and program staff.
  • Notify and document any changes in client condition to all appropriate parties.
  • Ensure accurate transcription and follow‑through of physician orders per policy.
  • Train clients, staff, and others on appropriate health information in a timely manner; share educational materials as needed.
  • Promote effective communication among all staff members and communicate pertinent health information to appropriate personnel.
  • Perform direct care utilizing established procedures, policies, and standards as evidenced by client/staff feedback.
Additional Responsibilities
  • Perform other duties as may be assigned.
Qualifications Education & Licensure
  • Valid Registered Nurse (RN) license in the State of California.
  • Current CPR/First Aid certification for Healthcare Professionals.
Experience
  • Experience providing services to individuals with intellectual and developmental disabilities preferred.
Knowledge & Technical Skills
  • Knowledge of HIPAA guidelines and patient privacy regulations (required).
  • Ability to read, write, and understand the English language.
  • Ability to multi‑task, prioritize, and manage time efficiently.
Interpersonal & Professional Skills
  • Able to establish and maintain effective, professional relationships with clients, coworkers, neighbors, volunteers, vendors, and the public.
  • Demonstrates patience and the ability to tolerate and navigate challenging or frustrating situations with professionalism and calm.
Additional Requirements
  • Ability to drive an agency vehicle safely, lawfully, and confidently in various traffic and weather conditions.
