Be the Best Hospice RN You Can Be. We are looking for a dedicated and compassionate RN to deliver high-quality hospice care and support patients and their families. The right candidate will:

• Conduct comprehensive nursing assessments and develop individualized care plans in coordination with the interdisciplinary team to address physical, emotional, social, and spiritual needs.
• Maintain timely and accurate documentation of care, interventions, and patient status updates, ensuring compliance with internal protocols and regulatory requirements.
• Coordinate care activities, including visit scheduling, medication management, physician collaboration, and interdisciplinary communication to assure continuity and quality of care.
• Provide education and support to patients and caregivers and respond to urgent needs during on‑call or admissions assignments.
• Supervise LPNs, LVNs, and hospice aides as needed, and participate in IDG meetings, patient conferences, and ongoing clinical education.
• Support pre‑hospice services by evaluating patient status via phone, telehealth, or in‑person visits and identifying additional care needs.
