DESCRIPTION

Under general supervision, the Nurse Practitioner (NP) is responsible for providing transplant patient care in the Liver Transplant Program. Provides clinical assistance and support in treating and managing outpatients and acts as key member of the multidisciplinary liver transplant team. Evaluates patients in the pre-transplant and post-transplant phases, including history and physical, patient preparation, procedures and management. Solves complex clinical problems and communicates directly with the attending physician, fellows and residents.

The NP is a consultant and liaison for patients and their families with attending physicians, house staff, social service staff, financial and administrative staff, inpatient nursing and community organizations. Provides teaching to patients, staff, and community organizations. Designated expert resource person on the basis of advanced clinical expertise in patient care for liver transplant patients. Assigned more complex cases requiring the use of advanced skills and typically works with greater independence.

MINIMUM QUALIFICATIONS
  • Must be a graduate from an accredited nurse practitioner program.

  • Current non-probationary California RN and NP licenses required or must be obtained prior to provider credentialing application and subsequent employment start date.

  • California Nurse Practitioner Furnishing License Certification prior to start date.

  • National NP board certification in aligned specialty with the practice area.

  • BLS/CPR certification at time of hire with commitment to get BART within six months of hire date.

  • Minimum two (2) years of recent experience as a practicing licensed Nurse Practitioner (NP). Experience must include Solid Organ Transplant or Hepatology, which may be met through clinical rotation or practicum.

  • Ability to communicate effectively with patients and other health care personnel, and skill at providing clear written, verbal and telephone communication with a wide variety of people.

  • Ability to maintain detailed medical information in an organized and easily retrievable manner.

  • Proven ability to manage comprehensive patient care with a strong emphasis on clinical decision-making, care coordination, and interdisciplinary collaboration.

  • Writing skills to prepare clear medical reports, laboratory data and progress notes, as well as policies, procedures and protocols for patient care management.

  • Ability to perform initial history, and physical examination, identify major medical problems, and formulate appropriate plans of action.

  • Demonstration of a high level of professionalism and excellent verbal and written communication skills. Ability to function well independently and as part of an interdisciplinary team.

  • Demonstrated professionalism, emotional resilience, and sound clinical judgment in high-acuity and fast-paced environments.

  • Ability to understand the medical as well as psycho-social needs of a patient population with liver disease.

  • Comfort in a group teaching setting and willingness to do community outreach.

  • Excellent assessment skills and ability to make clinically sound decisions in a fast-paced, constantly changing environment.

  • Ability to perform patient history, physical examination, identify major medical problems, and formulate appropriate plans of action.

  • Excellent organizational skills and demonstrated ability to prioritize well.

PREFERRED QUALIFICATIONS
  • Liver Transplant NP experience. Demonstrated knowledge of liver disease and transplantation, including knowledge of the pathology of the patient population, expected outcomes, and normal and abnormal lab results.

  • Ambulatory care experience.

  • Experience with Epic Ambulatory and Epic Phoenix

  • Experience leading or actively participating in Quality Improvement (QI) and/or Performance Improvement (PI) initiatives aimed at advancing clinical outcomes, operational workflows, or patient experience.
