Overview
MDS coordinator (Healthcare)

Responsibilities- Administer patient assessments and oversee the assessment process; set assessment schedules and assure assessments are done accurately and timely.
- Coordinate the care plan according to regulatory requirements.
- Create schedules for all Medicare and Medicaid; initiate Medicare coverage for newly qualified patients or issue denial letters; stay updated on changes in Medicare coverage and determine documents needed for Medicaid reimbursement.
- Direct the day-to-day functions of the nursing aides in accordance with current rules, regulations, and guidelines that govern the long term care facility.
- Ensure all nursing personnel comply with the facility’s written policies and procedures; periodically review the department’s policies, procedure manuals, job descriptions, and recommend revisions.
- Plan shifts, services, programs, and activities with assigned nursing staff and support personnel; ensure the Nursing Service Procedures Manual is current and reflective of day-to-day nursing procedures; ensure compliance with the manual’s procedures.
- Make written and oral reports/recommendations concerning shift activities; cooperate with other resident services to maintain the resident’s total regimen of care; participate in the facility’s quality assurance program for the nursing service department.
- Participate in facility surveys (inspections) conducted by authorized government agencies as requested; periodically review and update the resident’s discharge plan as required; assist in planning the nursing services portion of the resident’s discharge plan.
- Interpret department policies and procedures to personnel, residents, visitors, and government agencies as required; admit, transfer, and discharge residents as required; complete accident/incident reports as necessary.
- Coordinate charting and documentation: complete required recordkeeping forms/charts at admission, transfer, and discharge; encourage physicians to review and sign orders and progress notes; transcribe orders to resident charts; document nursing notes descriptively; complete accident/incident reports and forward to the Director; ensure discharge and diet information is properly communicated and documented.
- Drug administration: prepare and administer medications as ordered; verify resident identity; maintain narcotic records; manage drug/discrepancies and disposal per procedures; communicate automatic stop orders to attending physicians as required.
- Personnel functions: participate in evaluations, determine staffing requirements, and make recommendations regarding personnel actions; manage shift staffing and performance; supervise nursing personnel and ensure adherence to policies; conduct regular rounds; ensure staff are graduates of approved CNA programs or under licensed supervision.
- Enforce policies for residents, staff, visitors; maintain interdepartmental relationships to support resident care; create a warm, positive, and calm unit environment; review complaints and grievances and report to the Nurse Supervisor; follow facility procedures.
- Information security and privacy: safeguard user IDs/passwords and protect residents’ protected health information; report unauthorized access attempts and follow procedures for information security.
- Nursing care functions: inform staff of admissions and room assignments; participate in orientation for new admissions; coordinate diagnostics/therapies; consult with physicians; review care plans and ensure adherence to treatments; supervise CNA performance and ensure alignment with the care plan; implement restorative/rehabilitative programs; supervise infection control and safety practices; administer procedures, including use of restraints as required by policy; collect specimens and document vitals;
