Location:
Various patient homes throughout San Diego County (including El Cajon, La Mesa, Chula Vista, etc.).
Schedule:
Part-Time; consistent assignment potential based on patient needs and RN availability.
Reports To:
Clinical Supervisor
- Perform comprehensive skilled nursing visits according to physician orders and the established plan of care.
- Conduct initial patient assessments, ongoing monitoring, and necessary interventions.
- Manage and administer medications and treatments safely and accurately.
- Provide thorough patient and family education regarding diagnoses, treatment plans, and self-care management.
- Maintain accurate and timely documentation in the electronic health record (EHR) system.
- Collaborate with the home health team (PT, OT, HHA, etc.) to ensure coordinated care.
Qualifications:
- Active, unrestricted Registered Nurse (RN) license in the State of California.
- Minimum of one (1) year of recent clinical experience (Home Health experience is a strong plus!).
- Current BLS/CPR Certification.
- Must have reliable, insured transportation to travel to patient homes within San Diego County.
- Strong foundation in clinical assessment and professional communication.
