With a growing patient population and market share, the UC San Diego Health Care Management team is growing. The department is seeking an experienced Case Manager to join their leadership team. At the direction of the department manager, the Care Management Supervisor shares responsibility for day to day operation of the care management department including direct supervision of the staff. Facilitates patient centered care, interactive management and collaboration within the care management department, interprofessional team and the community that serves our patient population.
The Supervisor has primary responsibility facilitating patient flow. They demonstrate expertise and the ability to implement timely transitions of care with plans that are safe, effective and focused on preventing unplanned returns to the ED and or hospital. This role models the mission, vision, and core values of the organization; supports evidenced based practice to insure optimal outcomes.

Minimum Qualifications- A Bachelor's of Science Degree in Nursing.
- Registered Nurse license issued by the State of California.
- BLS/CPR certification at time of hire with commitment to get BART within six (6) months of hire date.
- Accredited Case Manager or Certified Case Manager (ACM or CCM) or will obtain within one (1) year of hire.
- A minimum of two (2) years of acute RN Case Management or RN Care Coordination experience working with patients in the acute care setting.
- A minimum of four (4) years of RN experience in acute care.
- Experienced with utilization review and/or appropriate utilization of health care resources.
- Must demonstrate effective communication skills with various stakeholders both verbally and in writing.
- MSN or equivalent.
- Expertise using Inter Qual Criteria.
- Demonstrated ability to effectively supervise a team and to manage the complex workflow and multiple priorities involved with care coordination and case management.
- Recent experience leading a team, project, or committee.
- Must be able to work various hours and locations based on business needs.
- Employment is subject to a criminal background check and pre-employment physical.
Pay Transparency Act:
Annual Full Pay Range: $132,600 - $259,800 (will be prorated if the appointment percentage is less than 100%). Hourly Equivalent: $63.51 - $124.43. Total pay scale is based on experience, licensure, certifications, and other business needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position.
The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and applicable collective bargaining agreements.
