Roles & Responsibilities   Evaluate patients and develop individualized physical therapy treatment plans that promote recovery and independence
Deliver skilled therapy to improve mobility, strength, balance, and functional outcomes
Collaborate with an interdisciplinary rehab team to create coordinated, patient-centered care plans
Educate patients and caregivers on exercises, safety, and mobility techniques to support long-term progress
Maintain accurate and timely documentation to ensure continuity of care and regulatory compliance
Qualifications   Active California Physical Therapist (PT) license required
SNF or rehabilitation experience preferred — bring your skilled nursing know-how
Strong clinical judgment with the ability to assess and adapt treatment plans
Excellent communication skills for working with patients, families, and the care team
Comfortable working independently and as part of a collaborative team
Knowledge & Skills   Patient evaluation and assessment for skilled nursing populations
Treatment planning and individualized intervention strategies
Therapeutic techniques to improve mobility, strength, balance, and function
Clear clinical documentation and charting practices
Effective interdisciplinary communication and teamwork
Patient and caregiver education for safe mobility and continued progress
