Recuperative Care Program (RCP)
The Recuperative Care Program (RCP) provides short-term medical and supportive care for individuals experiencing homelessness who are recovering from illness, injury, or hospitalization. By combining clinical care with housing-focused services, RCP helps clients stabilize their health and transition toward long-term recovery and housing stability.
The RoleAs a Registered Nurse (RN) in the Recuperative Care Program, you will provide direct clinical care and care coordination for individuals in a residential setting. You will work closely with interdisciplinary teams, including case management, residential services, and healthcare partners—to support clients’ medical recovery and overall well-being. This role is ideal for a compassionate, adaptable nurse who thrives in community-based care and values trauma-informed, client-centered practice.
YourRole in Supporting Health and Recovery
- Provide direct nursing care, including triage, medication administration, health assessments, wound care, and patient education in a residential setting.
- Monitor client health status and document care in the electronic health record (EHR), ensuring accurate and timely documentation.
- Coordinate care with internal teams and external providers, including hospitals, clinics, pharmacies, and specialty providers.
- Support intake, ongoing assessment, care planning, and discharge planning for recuperative care clients.
- Facilitate referrals and collaborate with health plans and community partners to ensure continuity of care.
- Respond to medical concerns and emergencies using trauma-informed and de-escalation approaches.
- Collect and track clinical data to support quality improvement and program outcomes.
- Complete required trainings and comply with clinical, organizational, and regulatory standards.
Minimum Requirements
- Graduate of an accredited nursing program.
- Active California Registered Nurse (RN) license or eligibility to take the National Council Licensure Examination (NCLEX).
- Current Cardiopulmonary Resuscitation (CPR), First Aid, and Automated External Defibrillator (AED) certification.
- At least six (6) months of nursing experience in clinical, community health, home health, medical-surgical, or similar healthcare settings.
- Basic to intermediate proficiency in Microsoft Office and electronic health record systems.
- Ability to collaborate effectively within interdisciplinary teams and communicate clearly with diverse populations.
- Ability to meet required health screenings and compliance standards (reasonable accommodation considered).
- Valid driver’s license preferred to support occasional travel between locations.
Preferred Experience
- Patient-centered, trauma-informed approach with strong clinical judgment and critical thinking skills.
- Ability to assess, prioritize, and respond effectively to multiple patient needs in dynamic environments.
- Strong documentation, organization, and care coordination skills, with attention to detail.
- Professionalism, empathy, and ability to de-escalate challenging situations with diverse populations.
- Comfort working in residential, community-based, or non-traditional healthcare settings.
- Experience working with vulnerable or underserved populations, including individuals experiencing homelessness or medically complex adults.
- Background in recuperative care, community health, home health, behavioral health, or public health.
- Familiarity with discharge planning, interdisciplinary team models, and quality improvement practices.
- Bilingual skills (preferred but not required).
This is a full-time position, typically 40 hours per week, scheduled Monday through Friday. This position may require flexible scheduling, including evenings, weekends, or holidays, based on program needs.
Compensation & BenefitsPay is based on experience, qualifications, and internal equity. The anticipated starting pay range for this role is $34.08 to $40.43 per hour.
