Position Summary

The official job title is Lead Care Manager (LCM). The Lead Care Manager (LCM) is a vital part of our Enhanced Care Management (ECM) program, coordinating care for individuals across various healthcare areas (physical, behavioral, dental, etc.). The LCM ensures clients receive the right care at the right time, empowering them to live successfully in their communities. Coordinates care and services for individuals in the ECM program, ensuring they receive appropriate care and support.

Core

Duties
  • Patient Engagement: Build rapport with patients, conduct needs assessments, and develop personalized care plans.
  • Care Coordination: Connect patients with essential services, including medical appointments, social services, and community resources.
  • Advocacy: Act as a liaison between patients and healthcare providers, ensuring timely access to care and addressing any barriers.
  • Education and Support: Provide health education, promote self-management strategies, and offer emotional support to patients and families.
  • Administrative Support: Assist with appointment scheduling, medication adherence, transportation arrangements, and other administrative tasks.
  • Communication: Maintain clear and effective communication with patients, families, and healthcare providers.
  • Documentation: Ensure accurate and timely documentation in accordance with program standards.
  • Teamwork: Collaborate effectively with the ECM team and other healthcare professionals.
  • In-Person visits: Conducts monthly in-person outreach and follow-up with patients.
  • Perform additional duties as assigned.
Qualifications
  • High School Diploma or GED required; healthcare experience preferred.
  • Valid California driver's license and insurance required.
  • Bilingual English/Spanish preferred.
  • Strong organizational and problem-solving abilities.
  • Knowledge of healthcare systems and community resources.
  • Experience with motivational interviewing and trauma-informed care.
  • 1-3 years of experience in community outreach, customer service, or healthcare.
  • Excellent communication, interpersonal, and organizational skills.
  • Knowledge of medical terminology.
  • Computer proficiency.

Pay range: $25 USD - $28 USD
