Job Summary

Provide support and guidance regarding pediatric medical and support services. Act as a liaison to improve patient care and flow between internal and external stakeholders.

Key Responsibilities
  • Facilitate care transitions, follow up on referrals, and ensure timely engagement.
  • Assist with enrollment in pediatric services, including scheduling and follow-up.
  • Communicate the importance of well-child visits to patients and families.
  • Accurately complete documentation and manage patient registry data.
  • Maintain records in CMIS and ensure compliance with standards.
  • Collaborate with healthcare teams to improve retention and address barriers to care.
  • Engage with patients via phone and in person to meet their needs.
  • Perform clerical tasks such as data entry and report generation.
  • Handle confidential information with discretion.
  • Perform other duties as assigned.
  • Minimum Requirements
    • High School diploma or GED.
    • Valid California driver’s license, car, and insurance; willingness to travel between sites.
    • Experience providing services to underserved populations.
    • Bilingual in Spanish/English required; trilingual preferred.
    • Strong communication, interpersonal, and organizational skills.
    • Basic computer literacy and knowledge of health/social service systems preferred.
    Compensation and Benefits

    Salary range: $22.00 - $26.17 per hour. Final salary based on experience, skills, and location. We offer a comprehensive benefits package.
