We are seeking a compassionate and detail-oriented PACE Home Care Coordinator to support our interdisciplinary team at the Chula Vista location. In this hybrid field/office role, you will coordinate unskilled home-based care services—including meal delivery, caregiver visits, and durable medical equipment—for seniors enrolled in the PACE program. You’ll advocate for participant needs, conduct home safety evaluations, and ensure services are delivered in alignment with participants goals and care plans.
Most days include 1-3 home visits (30-60 minutes each), with the remaining time in-office. This role is perfect for someone who values both independence and collaboration, and who finds fulfillment in delivering person-centered care to an underserved population.

- Paid holidays, sick leave, and vacation
- Mileage reimbursement for field visits
- Consistent daytime schedule—limited weekends or overnights
- Growth opportunities within a mission-driven, community-based organization
This opportunity is based in Chula Vista, a vibrant and diverse community in southern San Diego County. With coastal access, family-friendly neighborhoods, and close proximity to downtown San Diego, the area offers a rich lifestyle with affordable housing compared to central urban areas. You'll also be within reach of beaches, mountains, and cultural hubs—ideal for work-life balance.
Who Are We?We are a Federally Qualified Health Center deeply committed to delivering compassionate, affordable, and accessible healthcare for the entire family. Our organization supports over 100,000 patients through 40+ program sites, and the PACE program specifically helps seniors age in place while receiving wraparound care and services. We believe in teamwork, cultural humility, and advocacy—and we’re proud to be driven by mission, not margin.
