• Effectively screen and evaluate patients with cognitive and/or ADL physical functioning disorders and develop a plan of care following regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled and medically necessary treatment intervention to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administer various procedures as part of the rehabilitation plan including ADL retraining, therapeutic activities, therapeutic exercises, modalities, use of supportive and assistive devices, etc.
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluate treatment outcomes for effectiveness and modify services or refer as appropriate.
• Provide consultation and counseling to patients, families, caregivers and other service providers related to the physical disorders.
• Effectively communicate with supervisors and other health team members regarding patient progress, barriers and treatment plans.
• Provide comprehensive discharge summaries of services per regulatory and clinical requirements.
