Job Description:

* Develop effective treatment plans and obtain necessary approvals from referring physicians.
* Provide patient care according to the physician-approved treatment plan.
* Communicate regularly with supervisors and other healthcare team members regarding patient progress, challenges, and plans.
* Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed.
* Engage in in-service training programs for facility staff.
* Maintain accurate documentation of evaluations, daily treatment notes, weekly progress reports, and recertification documentation in accordance with the Speech and Language Pathology Board State Practice Act and regulatory requirements.
* Document treatment changes per facility policies and procedures.
* Educate patients’ families or nursing staff on maintenance programs and provide caregiver training in preparation for discharge.
* Participate in discharge planning to ensure a smooth transition for patients.
* Serve as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologists and student programs.
* Comply with the Speech-Language Pathology and Audiology Bureau State Practice Act and facility policies.
* Build positive, effective relationships with patients, residents, families, staff, and professional colleagues.

Required License or Certification:
