Del Rosa Post Acute is looking for a compassionate, organized, and energetic LVN Case Manager to join our care‑driven community. If you love coordinating care, supporting residents and families, and being the communication hub that helps everything run smoothly, this may be the perfect next step in your career.

In this role, you'll use your clinical background and critical‑thinking skills to guide residents through their care journey while partnering with an interdisciplinary team dedicated to exceptional outcomes.
What You'll Do

  • Communicate regularly with residents and families regarding Plans of Care, therapy programs, treatment needs, and expectations
  • Participate in morning stand‑ups and collaborate with nursing, therapy, admissions, and administrative teams
  • Answer resident and family questions to ensure they feel informed, reassured, and supported
  • Monitor changes in condition, reassess needs, and adjust plans accordingly
  • Organize daily workflow by prioritizing new admissions, current residents, and upcoming discharges
  • Complete required documentation with accuracy and timeliness
  • Perform utilization review activities to ensure timely, cost‑effective care
  • Coordinate admissions, discharges, and referrals
  • Work collaboratively with providers, insurers, and community resources to support seamless care transitions
