Nurse Practitioner - Triage Team

Responsibilities

The Nurse Practitioner - Triage Team provides advanced clinical support within the PACE center by evaluating urgent participant concerns, managing acute conditions, and coordinating care. This role ensures timely intervention and serves as a clinical leader for triage operations.

Essential Functions and Work Responsibilities

* Serve as the primary clinical resource for urgent participant needs within the PACE center.

* Perform rapid assessments, physical exams, and order/interpret diagnostic tests for triage cases.

* Formulate treatment plans, prescribe medications, and provide immediate interventions as needed.

* Manage participant and staff inquiries, prioritizing care and routing appropriately.

* Educate participants, caregivers, and staff on urgent care protocols and health management.

* Collaborate with interdisciplinary team members to ensure continuity of care.

* Document all triage encounters and interventions in compliance with Innov Age standards.

* Provide leadership in managing medical emergencies within the center.

Qualifications

REQUIRED

* Current State issued Nurse Practitioner license

* DEA Certification with full prescriptive authority

* 1+ year as a practicing Nurse Practitioner

* 5+ years a Registered Nurse with experience in elderly care

PREFERRED

* Bi-lingual

* Experience in acute care or urgent care settings.

$140,000 - $170,000
