Overview

Works within the vision, mission and philosophy of the agency. The Family Services Coordinator is responsible for developing and coordinating family-centered teams. Provides cross-functional team support to enhance the efforts of youth and their families/caregivers, system workers, community resources and natural supports in the creation of family driven, strengths based service plans. Provides care coordination and case management services including all related documentation for services planned and provided.

Works independently, with limited supervision, in family homes and a variety of community and/or treatment settings directly with children and adults.

Compensation We Offer
  • The initial compensation for this position ranges from $22.95 - $28.23 per hour; depending on experience, location and internal equity considerations
  • 8% License Differential for LMFT, LCSW, LPCC, PsyD. and select Peer Certifications**
  • 7.5% Bilingual Differential for qualified positions**

Schedule

Mon-Fri 9am-5pm; some weekends as needed, crisis support as needed

Responsibilities
  • Provides case management and coordination in the development of child and family teams (CFTs) and individualized services for children and families. Monitor services and treatment plan implementation. May require training in integrated Core Practice Model and high-fidelity wraparound.
  • Coordination of the assessment and treatment planning process and completion of the CANS. Effective communication so that everyone on the team (family team, community partners, inter- and intra-agency partners, coworkers and management) can participate in informed decision making.
  • Implements the 10-step strength-based family centered methodology in the wraparound planning process, with accompanying team building skills.
  • Assesses referring worker’s concerns, needs and expectations for service.
  • Elicits, appreciates and incorporates divergent perspectives in action plans and strategies in order to help youth stabilize in a placement and connect them with a permanent setting
  • Develops creative methods to ensure individualized culturally sensitive care.
  • Participates in family-find activities. And ensures the involvement of natural supports in the Child and Family Team.
  • Collaborates with Facilitators, Clinicians or Clinical Program Manager in the development of clinical assessments and integrated plan that is responsive to individualized needs, while building on youth and family strengths. Manages scope of practice with attention to coordination of clinical support.
  • Assesses youth and family needs using life domains. Supports the prioritization of life domains and needs by the Child and Family Team.
  • Provides meeting minutes to all Child and Family Team members to support communication, inclusion and accountability. Minutes reflect decisions, agreements, and outcomes of prior strategies.
  • Participates with families as a complete unit with focus on individual strengths/concerns/needs ensuring parent voice, choice and access.
  • Coordinates treatment plans and curriculum based therapeutic services for children.
  • In collaboration with Clinician or Clinical Program Manager, supports assessment of children, families and environments to identify behaviors and other factors that indicate safety risks (i.e., suicide, violence/aggression, substance abuse, use of medication, etc.) develops a risk specific reactive and proactive safety plan.
  • Collaborates with youth and family in identifying strengths and natural supports and resources.
  • In collaboration with Clinician/Clinical Program Manager and Family Specialists, develops and implements skill-building and strength-based intervention strategies for use with youth and families.
  • Provides leadership in responding to parents, children and staff when a crisis arises
