- Applicants must have attained the age of 18 years of age
- Associate degree plus 2 years of experience in a social service field OR Bachelor’s degree in a social science field plus at least 6 months experience in a social service field or a school internship in a social service field.
- License or Certification
The Case Manager position has primary responsibility for collaborating with the treatment team to facilitate the appropriate discharge and transfer of clients to the appropriate services and level of care. This position is responsible for participating in the treatment planning process that begins from initial assessment and unfolds throughout the course of treatment in the program through the collaboration with the client and BHUCC treatment team.
The case manager participates in interdisciplinary meetings, collaboration and communication with outside partners and resources, and with preparing clients for program transfer and discharge.
Valley Star’s Full‑Service Partnership offers mental‑health services and community support for adults ages 18 to 59. Care is provided in the office, at home, and in the community. Services include individual, family, and group counseling, psychiatric care, medication management, crisis support, intensive case management, housing and job assistance, and independent living skills training.

For Additional InformationIn accordance with California law, the grade for this position is 20.20 - 30.30. Placement within the grade is determined based on experience, internal equity, and other factors permitted by law.
