Preschool Services Coordinator (Case Management)

San Bernardino Preschool East Mountains and Desert, Inland Regional Center

Summary

Under the direct supervision of the Program Manager, the Coordinator is responsible for coordinating services and support available to consumers as outlined in the Lanterman Developmental Disabilities Services Act. The position provides services to consumers in the San Bernardino area, including driving to multiple service locations. No supervisory responsibilities.

Hourly Range

$25.67–$39.84

Key Responsibilities
  • Assist in daily Consumer Services Coordinator responsibilities under Program Manager direction.
  • Evaluate consumer progress and plan based on Individual Program Plan (IPP) on an ongoing basis.
  • Assure that consumers’ rights and dignity are maintained while providing services.
  • Maintain contact with consumers and their families as required.
  • Provide advocacy services, attend relevant meetings (e.g., IEP), and prepare paperwork.
  • Utilize and refer generic resources for consumers; provide information and referral services.
  • Conduct individual or family crisis intervention and document activities.
  • Complete special incident reports on a timely basis; route to appropriate programs/agencies.
  • Coordinate consumer assessment across nursing, medical, psychological/developmental, education or vocational disciplines.
  • Provide complete case coordination coverage when the assigned Consumer Services Coordinator is absent.
  • Coordinate, implement, and update the CDER, Annual Review and IPP process.
  • Serve Medicaid Waiver cases, completing required documentation timely and thoroughly.
  • Complete Placement and Liaison Activities as required.
  • Act as a liaison to at least two community agencies, attending meetings and acting as a resource.
  • Provide written annual reports of activities to the Program Manager.
  • Arrange placement of consumers in residential facilities or other programs as clinically indicated.
  • Deliver case coordination services in all levels of community care facilities.
  • Represent the regional center at case conferences and interagency meetings.
  • Maintain caseload status, community liaison activities, and case situation reports for the Program Manager.
  • Provide training to new employees, completing sponsorship checklist.
  • Lead or participate in at least one quality assurance evaluation or audit per year.
  • Complete all assignments from the agency’s Performance Contract.
  • Complete administrative requirements, including purchase of service forms and consumer file documentation.
  • Maintain 95% required case‑related paperwork completion within designated time frames.
  • Coordinate purchase of services identified in IPP, documenting requests in quarterly reports.
  • Prepare and present cases for compliance review, eligibility review, and other administrative reviews.
  • Complete Targeted Case Management (Title XIX) documentation accurately and timely.
  • Correct record deficiencies promptly.
  • Be available for transfer to another unit as determined by the Inland Regional Center.
  • Maintain interpersonal and community relationships at a high skill level.
  • Comply with the Agency’s Injury and Illness Prevention Plan and report unsafe conditions.
  • Contribute to the efficiency and training of the team.
  • Work cooperatively and effectively with others.
  • Use office equipment appropriately and maintain a tidy work area.
  • Handle change well, remain flexible and adaptable in dealing with new priorities and assignments.
  • Keep manager informed of pending work and problems encountered.
  • Maintain good attendance and punctuality.
  • Utilize agency’s IT systems securely and follow protocols.
  • Ensure program compliance with agency Personnel Policies and Procedures.
  • Stay current and expand relevant knowledge base; attend training sessions as required.
  • Organize travel efficiently; maintain a safe driving record.
  • Utilize bilingual skills where required.
  • Perform other work as assigned.
