SUMMARY: Under the direct supervision of the Program Manager, the Consumer Services Coordinator (CSC) is responsible for coordinating the services and supports that are available to persons with developmental disabilities and their families in accordance with the Lanterman Developmental Disabilities Services Act. This position has no supervisory responsibility.
This position would provide services to consumers in the San Bernardino Upper Desert Adult area, which includes driving to upper desert areas, Victorville, Apple Valley, Hesperia, Oak Hills, Adelanto, Phelan, and Pinon Hills. Mileage reimbursement for business travel is paid out on a monthly basis.

- Provide CSC daily case coordination services for consumers.
- Utilize Person-Centered Planning to create and execute the IPP for consumers, generate progress reports, and conduct annual reviews.
- Evaluate progress and plan as identified in the IPP; create and implement IPP Addendums as needed.
- Provide advocacy services to consumers and families; attend relevant meetings (IEP, ITP, IHC, etc.) and prepare paperwork as necessary.
- Seek and utilize generic resources on behalf of consumers and families.
- Complete individual or family crisis intervention and document activities.
- Complete Special Incident Reports timely and inform representatives of other agencies as appropriate.
- Coordinate consumer assessments (medical, psychological, developmental, educational, vocational, OT/PT, etc.).
- Complete placement and liaison activities as required.
- Ensure Medicaid Waiver standards are maintained and documentation is timely.
- Arrange placement of consumers in residential facilities, day programs, or other training programs as clinically indicated and approved by the Interdisciplinary Team.
- Complete facility audits (ICRC 513 form) as required and when issues are identified.
- Identify situations in vendored programs needing attention and make appropriate referrals to QA or Transportation programs.
- Keep the manager informed of caseload status, community activities, and difficult cases.
- Participate in case conferences and interagency meetings as needed.
- Provide case coordination coverage as directed if a CSC is absent or unassigned; mentor new employees as directed.
- Complete IRC orientation and new staff training.
- Visit Level 2–4 residential facilities and day programs as assigned.
- Participate in at least one Quality Assurance evaluation/audit annually.
- Complete administrative requirements daily, maintain consumer file information, and document activity in Target Case Management (Title 19) notes; meet minimum units and timely paperwork requirements.
- Facilitate the purchase of services identified in the IPP.
- Be prepared for compliance and eligibility reviews and other administrative case reviews.
- Maintain and expand knowledge; attend mandatory trainings and required CEUs.
- Collaborate as a team member, share expertise, adapt to change and manage a hybrid work environment (office, remote, field).
- Organize travel to visit consumers, families, and vendors efficiently.
- If bilingual, use skills as required.
- Use office equipment responsibly; keep area neat and comply with safety rules and IRC’s Injury and Illness Prevention Plan.
- Use IRC IT systems securely and in accordance with policies.
- Respect consumers’ rights and dignity in service provision; comply with IRC policies.
