Personal Care Assistant – Welbe Health PACE

Welbe Health PACE helps seniors stay in their homes and communities by providing comprehensive medical care and community-based services. We serve the most vulnerable seniors with better quality and compassion in a value‑based model.

The Person al Care Assistant is a vital part of our team, providing aid to our participants and contributing to the care planning process as a member of the interdisciplinary team (IDT). Reporting to the Day Center Manager, the assistant delivers safe personal care services at the Welbe Health day center.

• Assist participants with activities of daily living (ADLs) such as toileting, bathing, dressing, grooming, laundry, and physical lifting during transfers.
• Measure and document vital signs as instructed.
• Act as a medical escort for boarding and exiting transportation vehicles.
• Oversee activity on the day‑center floor to ensure participant safety.
• Assist with meal service, including feeding when needed.
• Maintain personal care supplies such as incontinence and grooming items.

• High school diploma or equivalent.
• Certification as Home Health Aide (HHA) or Certified Nursing Assistant (CNA) in California preferred.
• Minimum one year of personal care experience with seniors.
• CA Food Handlers Certification within 30 days of hire.
• CPR and First Aid certification within 180 days of hire (preferred).

$21.00 – $25.22 hourly plus bonus.
