Position Summary
Under the direct supervision of the Program Manager is responsible for coordinating the services and support available to consumers as outlined in the Lanterman Developmental Disabilities Services Act. Perform related work as assigned. Has no supervisory responsibilities.
This position would provide services to consumers in the San Bernardino Preschool West End area including driving to Chino, Chino Hills, Ontario, Guasti, Montclair, Rancho Cucamonga, Upland, and Fontana. Mileage reimbursement for business travel is paid out monthly.
Hourly Rate: $25.67 - $39.82.
Sign-On Bonus- $250 sign‑on bonus after completion of 90 days of employment at IRC with work performance in good standing.

- Assume daily Consumer Services Coordinator responsibilities guided by the agency’s and program’s mission statement and core values under the Program Manager’s direction.
- Evaluate consumer progress and plan as identified in Individual Program Plan (IPP) on an ongoing basis.
- Assure that consumers’ rights and dignity are maintained in the provision of services.
- Maintain contact with consumers and their families as required.
- Provide advocacy services; give high priority to attending relevant meetings (IEP) and prepare paperwork and notate the IPP as necessary.
- Seek out and effectively utilize generic resources on behalf of consumers; provide information and referral services.
- Provide individual or family crisis intervention and document activities under the Program Manager’s direction.
- Complete special incident reports and inform representatives of other programs/agencies as appropriate in a timely manner.
- Coordinate consumer assessment including nursing, medical, psychological/developmental, education or vocational and others as needed; follow up as required.
- Provide complete case coordination coverage to any consumer whose Consumer Services Coordinator is absent.
- Coordinate, implement and update the CDER, Annual Review and IPP process.
- Serve Medicaid Waiver cases by completing required documentation on time and thorough.
- Complete Placement and Liaison Activities as required.
- Act as the program’s/regional center’s liaison to at least two community agencies, parent groups or programs; attend meetings, provide information and serve as a resource.
- Provide written reports of activities to the Program Manager at least annually.
- Arrange placement of consumers in residential facilities or other programs in consultation with Resource Development, Transportation Program and Quality Assurance Program.
- Provide case coordination services to consumers in all levels of community care facilities.
- Represent the regional center at case conferences and interagency meetings.
- Keep the Program Manager informed of the caseload status, community needs, liaison activities, and unusual or difficult case situations.
- Act as program sponsor for new employees by providing training in agency philosophy, mission, values, policy and procedures.
- Lead or participate in at least one Quality Assurance evaluation or audit of a community care facility, health care facility, or day program annually.
- Complete all assignments arising from the agency’s Performance Contract.
- Complete daily administrative requirements, purchase of service forms and consumer file record documentation per agency policies and procedures.
- Achieve at least 95% completion of required case‑related paperwork within designated time frames.
- Coordinate purchase of services identified in IPP and document requests in quarterly reports.
- Prepare and present cases for compliance review, eligibility review and other administrative case reviews.
- Complete Targeted Case Management (Title XIX) documentation accurately and timely, securing all possible units but not less than the agency minimum set by management.
- Correct out‑of‑date records promptly.
