Associate Therapist (AMFT, ACSW, APCC) – Contract-to-hire

Pay:$33.65 – $36.06/hour
Schedule:Full-Time | 8am - 5pm, Monday - Friday | Hybrid
Join a mission-driven behavioral health team working to break the cycle of homelessness and mental illness. We’re hiring an
Associate Therapist (AMFT, ACSW, or APCC)to provide field-based therapy, intensive case management, and housing support to adults with chronic mental health conditions in San Bernardino County.
This is a full-time, hybrid role that combines clinical care, community engagement, and wraparound services using Housing First and recovery-oriented models.
What You’ll DoAs an Associate Therapist, you’ll serve as the clinical lead for a caseload of clients living in Permanent Supportive Housing. You’ll work alongside a multidisciplinary team providing trauma-informed care in homes, shelters, clinics, and other field settings.
Clinical & Case Management ServicesProvide individual therapy, crisis intervention, and rehabilitative support
Complete intake assessments, treatment plans, and clinical documentation
Collaborate with psychiatrists and licensed staff to develop client goals
Monitor mental health symptoms and intervene proactively
Facilitate linkages to psychiatric, medical, housing, and recovery resources
Conduct outreach and home visits to ensure consistent engagement
Housing & Stability SupportAssist clients with housing applications, benefits, and documentation
Guide clients through lease-up and maintain compliance with housing requirements
Promote independent living skills and community reintegration
Use harm reduction and motivational interviewing to support behavior change
Attend weekly team meetings and clinical supervision
Participate in 24/7 on-call rotation (shared across the team)
Maintain timely and compliant documentation in EHR (Welligent)
Meet productivity and billing standards in accordance with Medi-Cal
What We’re Looking ForRegistered with the CA BBS as an
AMFT, ACSW, or APCC
Master’s degree in Social Work, Counseling, Psychology, or related field
1–2 years’ experience working with adults with serious mental illness or homelessness preferred
Strong grasp of trauma-informed care, recovery principles, and harm reduction
Ability to work independently in the field and manage dynamic caseloads
Valid CA driver’s license, personal vehicle, and current auto insurance
This is a
hybrid field-based position
serving clients across
San Bernardino County
. Regular driving is required. You’ll work with individuals in various settings including homes, shelters, clinics, and the community.
