Position: Family Service Case Manager (TREE Program)

The Family Service Case Manager is responsible for providing outpatient treatment and wraparound services to 9th-12th grade High School students with substance use disorders and their families.

REPORTS TO: Youth Prevention Director

Core Duties
  • Complete assessments for participants referred to project.
  • Screen parents and families to ensure that required program criteria are met.
  • Create service plans for participants and families enrolled in project.
  • Create referrals and follow ups to other social services for participants as part of their continuum of care.
  • Provide case management for families to ensure wraparound services are obtained.
  • Attend staff meetings and collaborate with the Director on process, outcomes, evaluation, and the development of activities to meet program goals.
  • Integrate strength and evidence-based practices into daily interactions with participants, parents, and families.
  • Provide responsible documentation of services provided in client records.
  • Work independently in community settings with flexible working hours.
  • Attend ongoing training as required by the Director.
  • Other duties as assigned.
Education/Work Experience
  • Bachelor's degree in the field of psychology, social and/or human services, or related discipline.
  • A minimum of 3 years demonstrated experience in client case-management and experience in conducting presentations, group facilitation, training, or teaching adults.
  • A minimum of 3 years demonstrated experience in working with families.
  • Extensive knowledge of chemical dependency.
  • Experience working with culturally diverse populations.
  • Excellent oral, written, and case management skills required.
Skills and Knowledge
  • The Stage of Change Theory, Motivational Interviewing, Contingency Management, and Cognitive Behavioral Techniques.
  • Behavioral problem-solving techniques for addiction treatment.
  • Principles of recovery from SUD or mental illness.
  • Case Management activities and knowledge of community resources for social support.
  • Knowledge of mutual-help groups, their functions, values/beliefs and how to access.
