Position: LICENSED VOCATIONAL NURSE-Long term care

Description This is a 12-hour shift day position with rotating weekends. We are seeking a compassionate and clinically strong Nurse (LVN) to provide person-centered care in our Green House home setting. This role partners closely with a self-managed team to ensure each elder’s clinical needs are met with dignity and excellence. Our long-term care facility will be in the newly built Green house homes.

There are four homes that have 12 rooms each and there is a nurse for every two homes.

Job Details This is a 12-hour shift day position with rotating weekends. We are seeking a compassionate and clinically strong Nurse (LVN) to provide person-centered care in our Green House home setting. This role partners closely with a self-managed team to ensure each elder’s clinical needs are met with dignity and excellence. Our long-term care facility will be in the newly built Green house homes.

There are four homes that have 12 rooms each and there is a nurse for every two homes. Nurses role on the CST (Clinical Support team) includes assessing and monitoring the clinical status of the elders and delivering evidence-based clinical care, both directly and through the Care Partners and other CST members. The nurses report directly to the Director of Nursing but work in collaboration with the Guide and Care Partners.

Key Responsibilities
  • Assess and monitor each elder’s clinical status during every shift
  • Administer medications and treatments
  • Manage pain, monitor skin integrity, nutrition, hydration, continence, mobility, and mentation
  • Ensure care plans are current and properly implemented
  • Support MDS, infection control, and care planning activities
  • Provide education to team members, elders, and families
  • Maintain emergency supplies and follow infection control standards
  • Mentor and support the self-managed work team (SMWT)
Qualifications
  • Active Texas LVN license
  • Current BLS/CPR certification
  • Minimum 2 years nursing experience (skilled nursing or long-term care preferred)
  • Strong clinical assessment and critical thinking skills
  • Experience with EMR systems (we use PCC)
  • Passion for person-centered care and team collaboration
Work Environment
  • Green House home setting focused on resident-directed care
  • Requires lifting/transferring residents (up to 50 lbs with assistance)
  • Exposure to healthcare environments and PPE use
  • Flexible scheduling, including weekends and holidays
