Responsibilities
  • Provide and document skilled nursing care under the supervision of a Registered Nurse, following the care plan and physician orders
  • Observe, assess, and report changes in patient condition to supervisor or physician as appropriate
  • Document activities, assessments, nursing actions, responses, and coordination of care in a timely manner
  • Participate in, implement, and update the nursing care plan
  • Take appropriate nursing action based on assessment to achieve expected outcomes
  • Recognize changes in patient needs requiring intervention and implement risk-reducing care
  • Adhere to Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act
  • Provide care with infection control measures per OSHA standards
  • Educate patients and families on disease processes, self-care techniques, and prevention strategies
  • Maintain knowledge of nursing competencies through continuing education and skill evaluations
Qualifications
  • Graduate of an accredited school of nursing
  • Current, unrestricted LPN/LVN license in the state of practice
  • Current CPR certification
  • Demonstrated proficiency in clinical assessments, documentation, and adherence to nursing policies and procedures
State-Specific Requirements
  • South Carolina – One year of pediatrics experience
  • California – One year of experience required working under current nursing license
  • Louisiana – One year of experience required working as a licensed nurse
  • Continuing Education as required by state
Preferences
  • Six months of recent experience as a licensed nurse in a clinical care setting
  • Home health experience

  • Must be able to speak, write, read, and understand English
  • Must be able to travel

  • Must be able to sit and climb stairs
  • Must have visual and hearing acuity
  • Must have strong sense of smell and touch
  • Must be able to respond appropriately to emergency situations in the home or during transport
