LVN - The Mission M - F 2p - 10p – Blue Skies of Texas
Join our team at the Blue Skies Mission - San Antonio, TX and make a difference in the lives of our residents. We are seeking a dedicated Licensed Vocational Nurse (LVN) to serve as a charge nurse in our Senior Living Community, working the evening shift (M-F 2 pm – 10 pm). The position is full‑time and supervised by the Director of Nursing.

- Supervise and provide nursing care in accordance with physician orders and recognized nursing techniques.
- Perform duties related to pre‑admission, admission, and discharge of residents.
- Manage the medication regime, including acceptance of physician orders, transcription, and ensuring medications are on hand.
- Ensure medical supplies are available and request additional supplies in a timely manner.
- Maintain a safe environment for residents.
- Supervise Health Support Assistants (HSAs) and ensure they provide appropriate care.
- Participate in Quality Assurance and Performance Improvement Committees.
- Communicate with family members, physicians, staff, support services, and outside agencies about residents’ care and condition.
- Document observations, treatments, and medications in the resident Electronic Medical Record (EMR) and inform the supervisor of any adverse changes.
- Perform additional duties as assigned by the supervisor.
- High school diploma or equivalent.
- Must be a licensed LVN in the State of Texas and in good standing with the Board of Nurse Examiners.
- Current CPR, AED certifications and CEU requirements for licensure.
- Preferred:
At least one year of experience in a long‑term care setting. - Preferred:
Additional education beyond the minimum nursing licensure level, including training in supervision and infection control.
