Overview

Position: Integrated Care Nurse Practitioner (Hybrid)

Schedule: Monday–Friday, 8:00 AM–5:00 PM

Hybrid Role: 1–2 days telehealth (remote) + 3–4 days in patients' homes (varies weekly)

No Weekends | No On-Call

Compensation: Base salary +
15% quarterly bonus

We are seeking experienced Nurse Practitioners and Physician Assistants to join an integrated home-based care team serving patients with chronic and complex medical conditions, including kidney disease. Youll work alongside a Registered Nurse and Social Worker to deliver personalized, evidence-based care through a combination of in-home visits and telehealth.

• Conduct comprehensive assessments and annual wellness visits (home and virtual)
• Develop, implement, and adjust individualized treatment plans
• Prescribe medications and order diagnostic testing
• Coordinate care with PCPs, specialists, and ancillary services
• Educate patients and families on disease management and preventive care
• Maintain timely, accurate clinical documentation
• Participate in integrated care team meetings
• Follow up and evaluate patient progress to improve outcomes

• Active, unrestricted NP or PA license
• Board certification (NP: ANCC/AANP | PA: NCCPA)
• Active DEA license
• 2+ years of direct patient care experience
• Experience with annual wellness visits (home or virtual preferred)
• Ability to practice autonomously
• Valid driver s license, reliable transportation, and insurance
• Managed Care / IPA / Health Plan experience preferred
