Our Local Office is seeking compassionate Licensed Practical / Vocational Nurses to provide care for a very special client/patient. This position is part time with available shifts Monday and Friday (7a-7p) located in San Antonio, TX 78259. The role supports clients with feeding tube & medication administration. Training will be provided.

Position Overview

The Licensed Practical Nurse (LPN/LVN) is responsible for providing and documenting skilled nursing care under the supervision of a Registered Nurse in accordance with the developed care plan and physician orders. The role adheres to confidentiality standards and professional boundaries at all times.

Essential Job Functions
  • Deliver and coordinate quality patient care in compliance with physician orders.
  • Observe and assess patient condition continuously and report changes to supervisor and/or physician as appropriate.
  • Document all activities, assessments, nursing actions, responses and coordination of care promptly.
  • Participate and update the nursing care plan.
  • Implement nursing action based on assessment and achieve expected outcomes.
  • Recognize changes in patient needs and interventions to prevent or reduce risk.
  • Comply with Aveanna policies, state and federal regulations, accrediting bodies and the Nurse Practice Act.
  • Apply infection control measures that protect staff and patient according to OSHA standards.
  • Educate patient and family about disease process, self‑care techniques and prevention strategies.
  • Maintain knowledge of nursing competency through education and skill evaluation.
Qualifications
  • Active, unencumbered LPN/LVN license in the state where the clinician will practice.
  • Compact license must be transferred to state of residence within 90 days.
  • Current CPR certification with hands‑on component;
    Aveanna can assist in obtaining after hire.
  • TB skin test current within last 12 months.
  • Six months prior hands‑on nursing experience preferred but not required.
  • Reliable transportation.
Additional State Specific Requirements
  • South Carolina – One year of pediatrics experience.
  • California – One year experience working under current nursing license.
  • Louisiana – One year experience working as a licensed nurse.
  • Continuing Education as required by state.
Preferences
  • Six months of recent experience as a Licensed Nurse.
  • Home health experience.

  • Strong sense of smell and touch.
  • Respond physically and mentally to emergency situations in home or during transport.

  • Function in variety of environments including exposure to allergens.
  • Possible exposure to blood, bodily fluids and infectious diseases.
