Position OverviewThe Licensed Practical Nurse (LPN/LVN) delivers and documents skilled nursing care under the supervision of a Registered Nurse in accordance with individualized care plans and physician orders while maintaining confidentiality and professional boundaries.
Essential Job Functions- Coordinate and deliver high‑quality patient care in compliance with physician orders.
- Continuously monitor and assess patient condition, reporting changes promptly to supervisors or physicians.
- Document all nursing activities, assessments, actions, and care coordination in a timely manner.
- Participate in, implement, and update nursing care plans as needed.
- Take appropriate nursing action based on assessments to achieve expected outcomes.
- Identify changes in patient needs or responses requiring intervention and implement preventive measures.
- Adhere to Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Follow infection control practices that protect staff and patients according to OSHA standards.
- Educate patients and families about disease processes, self‑care techniques, and prevention strategies.
- Maintain proficiency through continuing education, internal learning management, and skill evaluations.
- Active, unencumbered LPN/LVN license in the state of practice.
- Compact licenses transferred to the state of residence within 90 days.
- Current CPR certification Deutschen hands‑on component;
Aveanna can assist after hire. - TB skin test current within the last 12 months.
- Preferred six months of prior hands‑on nursing experience, not required.
- Reliable transportation.
- Graduate of an accredited nursing school.
- Current, unrestricted state license for practice.
- Current CPR certification.
- Proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.
- South Carolina – one (1) year of pediatrics experience.
- California – one (1) year of experience under a current nursing license.
- Louisiana – one (1) year of experience as a licensed nurse.
- Continuing education as required by state regulations.
- Six (6) months of recent experience as a Licensed Nurse in a clinical setting.
- Home health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organization and communication skills.
- Speak, write, read, and understand English.
- Travel as needed.
- Lift 50 pounds.
- Reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Climb stairs and sit as required.
- Basic visual and hearing acuity.
- Strong sense of smell and touch.
- Respond physically and mentally to emergencies in the collaborated environment.
- Work in diverse environments, possibly exposed to allergens and various conditions.
- Potential exposure to blood, bodily fluids, and infectious diseases.
