Position Overview

The Licensed Practical Nurse (LPN/LVN) delivers and documents skilled nursing care under the supervision of a Registered Nurse in accordance with individualized care plans and physician orders while maintaining confidentiality and professional boundaries.

Essential Job Functions

• Coordinate and deliver high‑quality patient care in compliance with physician orders.
• Continuously monitor and assess patient condition, reporting changes promptly to supervisors or physicians.
• Document all nursing activities, assessments, actions, and care coordination in a timely manner.
• Participate in, implement, and update nursing care plans as needed.
• Take appropriate nursing action based on assessments to achieve expected outcomes.
• Identify changes in patient needs or responses requiring intervention and implement preventive measures.
• Adhere to Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
• Follow infection control practices that protect staff and patients according to OSHA standards.
• Educate patients and families about disease processes, self‑care techniques, and prevention strategies.
• Maintain proficiency through continuing education, internal learning management, and skill evaluations.

Qualifications

• Active, unencumbered LPN/LVN license in the state of practice.
• Compact licenses transferred to the state of residence within 90 days.
• Current CPR certification Deutschen hands‑on component;
  Aveanna can assist after hire.
• TB skin test current within the last 12 months.
• Preferred six months of prior hands‑on nursing experience, not required.
• Reliable transportation.

Requirements

• Graduate of an accredited nursing school.
• Current, unrestricted state license for practice.
• Current CPR certification.
• Proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.

Additional State Specific Requirements

• South Carolina – one (1) year of pediatrics experience.
• California – one (1) year of experience under a current nursing license.
• Louisiana – one (1) year of experience as a licensed nurse.
• Continuing education as required by state regulations.

Preferences

• Six (6) months of recent experience as a Licensed Nurse in a clinical setting.
• Home health experience.

Physical Requirements

• Speak, write, read, and understand English.
• Travel as needed.
• Lift 50 pounds.
• Reposition patients and move equipment without assistance.

• Climb stairs and sit as required.
• Basic visual and hearing acuity.
• Strong sense of smell and touch.
• Respond physically and mentally to emergencies in the collaborated environment.

Environment

• Work in diverse environments, possibly exposed to allergens and various conditions.
• Potential exposure to blood, bodily fluids, and infectious diseases.
