MDS Coordinator

Position Summary

We are seeking a detail-oriented and compassionate MDS Coordinator to join our healthcare team. In this role, you will oversee the resident assessment process and play a key role in ensuring high-quality care, regulatory compliance, and accurate reimbursement. Reports To: Health Care Administrator

FLSA Status: Exempt

• Manage and oversee the MDS assessment process, ensuring accuracy, completeness, and timely submission
• Establish and maintain assessment schedules in compliance with federal and state regulations
• Review and transmit assessments to the Department of Health
• Analyze assessment data and collaborate with leadership to recommend policy and process improvements
• Coordinate interdisciplinary care plans to support effective, person-centered resident care
• Oversee Medicare and Medicaid assessment schedules and coverage determinations
• Initiate Medicare coverage for eligible residents and issue denial notifications when required
• Stay current on Medicare and Medicaid regulations and assist with documentation needed for reimbursement

• Licensed Registered Nurse preferred
• Bachelor’s degree in Nursing
• Clinical experience in a healthcare setting
• Proficiency with MDS 3.0
• Strong computer and documentation skills
• Excellent communication and organizational skills
• Knowledge of federal and state healthcare regulations
