Position: Medical Assistant (MA)-Chronic Care Management

Overview

Job Purpose We are seeking a reliable and patient-focused Medical Assistant (MA) located in San Antonio, Texas to support our Telephonic Chronic Care Management (CCM) Program. This role involves working closely with patients who have multiple chronic conditions by conducting non-face-to-face outreach and helping coordinate ongoing care. The MA will support clinical staff by gathering information, providing education, and helping patients navigate their care journey remotely.

Essential

Job Duties & Responsibilities
  • Perform monthly telephonic outreach to patients enrolled in the CCM program to check in on their health and wellness.
  • Assist patients in following their chronic care plans, including scheduling appointments, medication refills, and lifestyle management.
  • Document 20+ minute non-face-to-face care management interactions per CMS guidelines.
  • Provide patients with general education and support under supervision of a PCP.
  • Identify and escalate health concerns to the PCP as appropriate.
  • Help patients overcome barriers to care, such as access to medication, transportation, or social services.
  • Use electronic health records (EHRs) to document patient interactions and update care plans.
  • Support population health and care coordination efforts within the practice or healthcare organization.
  • Manage a caseload of 200+ patients per month
  • Other duties as assigned.
Experience
  • Must reside in Texas and be familiar with Texas-based healthcare regulations.
  • Minimum 1–2 years of experience in a clinical or telehealth setting; experience with chronic care preferred.
Education
  • Certified or Registered Medical Assistant (CMA/RMA) – completion of an accredited MA program required
Knowledge, Skills & Abilities
  • Strong knowledge of chronic conditions like diabetes, hypertension, asthma, COPD, CHF, etc.
  • Excellent communication, phone etiquette, and interpersonal skills.
  • Comfortable using telehealth technology, EHRs, and Microsoft Office applications.
  • Bilingual in Spanish and English is required
Work Hours, Travel Requirements
  • Monday – Friday, 8:00 a.m. – 5:00 p.m., and as needed to complete projects.
  • Travel to medical offices may be necessary for the purpose of providing education.

Physical Requirements
  • This job operates in an office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, scanners, filing cabinets and fax machines.
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to talk and hear. This is largely a sedentary role; however, some filing is required. This would require the ability to lift files, open filing cabinets and bend or stand on a stool as necessary.

    Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus.
