POSITION SUMMARY/RESPONSIBILITIES
The Behavioral Health (BH) Social Worker assists Community First members with their Behavioral Health needs by assessing and identifying unmet needs and coordinating care, utilizing Covered Services and community resources with the goal of optimizing the Member's well-being, independence, and integration in the community. The BH Social Worker collaborates with other teams in Population Health Management (PHM) to ensure that the member's physical, behavioral, functional, and psychosocial needs and preferences are met.
BH Social Worker ensures adherence to all regulatory and accreditation requirements and follows Community First policies, procedures, and protocols.
EDUCATION/EXPERIENCE
A minimum of three years of social work experience is preferred. Master's degree in Social Work from a school accredited by the Council on Social Work Education is required. Two years' experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions is preferred. Knowledge of Medicare, Medicaid, community resources, alternate funding programs, Texas Health and Human Services Commission and Texas Department of Insurance standards and NCQA in a managed care setting is preferred.
LICENSURE/CERTIFICATION
Current unrestricted state licensure from the Texas State Board of Social Work Examiners is required.
