Master Social Worker PRN

Position Summary:

We are seeking a PRN Master Social Worker (MSW) to join our hospice care team. The ideal candidate will provide emotional, psychosocial, and resource-based support to patients and families, helping them navigate end-of-life care with dignity and understanding.

• Assess patients’ and families’ psychosocial, emotional, and support needs.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Provide counseling, crisis intervention, and grief/bereavement support.
• Assist families in accessing community resources and support services.
• Maintain accurate and timely documentation in accordance with state, federal, and agency guidelines.
• Participate in care conferences, team meetings, and patient/family education.

• Master’s degree in Social Work (MSW) from an accredited program.
• Current Texas licensure as LMSW.
• At least one year of hospice or palliative care experience preferred.
• Strong communication, counseling, and advocacy skills.
• Ability to work independently and collaboratively in a team setting.
