JOB DESCRIPTION:

• Medication review:
  Reviewing patient medication charts to identify potential risks and clarify information
• Educating patients:
  Informing patients about their medications, including dosage, side effects, and potential interactions
• Monitoring patient progress:
  Keeping track of patient outcomes and ensuring they don't experience negative side effects
• Advising healthcare professionals:
  Providing drug information and education to other healthcare professionals
• Collaborating with healthcare teams:
  Working with doctors, nurses, and other healthcare professionals to ensure optimal patient care
• Developing guidelines:
  Collaborating with the infection control team to develop tools and guidelines for the safe and effective use of antibiotics
• Assessing pharmaceutical treatments:
  Evaluating the results of pharmaceutical treatments
• Maintaining medication profiles:
  Keeping medication profiles for patients, including a registry of poisons and controlled drugs
