Saint Luke's Hospital invites qualified professionals to apply for the Clinical Pharmacist position. This role supports patient care through medication management, compounding, and clinical consultation.
Responsibilities- Interpret medication orders (verbal and written) and transcribe them to computerized patient medication profiles.
- Maintain accurate patient medication profiles.
- Compound and dispense pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition products.
- Dispense controlled substances to patient‑care areas and maintain records.
- Manage formulary by minimizing non‑formulary procurements and promoting rational drug therapy.
- Provide direction to pharmacy personnel and verify the daily activities of technicians, interns, and residents.
- Ensure compliance with regulatory agencies (TJC, TX State Board of Pharmacy, TX Department of Health, OSHA, DEA, TX Dept. of Public Safety, EEOC/TEC).
- Ensure safe, cost‑effective medication therapies for patients through policy‑based protocols.
- Detect and report suspected adverse drug reactions and other medication occurrences.
- Utilize formulary initiatives and clinical review to provide adequate pain management.
- Apply principles of drug dosing in pediatric and neonatal patients.
- Monitor drug therapy regimens for contraindications, drug‑drug and drug‑food interactions, allergies, and drug appropriateness.
- Adjust dosages or drug choice based on renal function in elderly or other compromised patients.
- Assist with pharmacokinetics consult service and drug dosing per BHS protocol.
- Review and interpret culture and susceptibility data; recommend antibiotic changes when needed.
- Provide clinical consultations and clarification to practitioners and suggest cost‑effective therapeutic alternatives.
- Offer drug information to professional staff, patients, and families.
- Participate in development and presentation of orientation, education, and training programs.
- Follow up with staff to resolve unmet interventions.
- Screen order entry to identify and document all clinical activities and interventions.
- Utilize resources efficiently and maintain a safe environment.
- Address population‑specific needs and participate in performance improvement activities.
- Minimum Experience:
Hospital Internship Rotation. - Preferred Experience:
Hospital experience. - Required Certifications/Licensure:
Current Texas State Board of Pharmacy license. - Preferred Certs/Licensure: IV Admixture Certification.
