Job Summary

We are searching for our next “STAR.” As the Registered Dietitian you will play an integral role in health behavior and interventions, contributing to the prevention and improved management of chronic disease. You will assess, evaluate, implement, and document nutritional interventions for Chronic Disease Prevention Program participants, providing nutrition assessments and information through individual consultations and group classes.

Duties and Responsibilities
  • Collaborate with primary‑care providers.
  • Document information in the patient chart, implement a plan, provide nutrition counseling, and make appropriate referrals.
  • Conduct monthly performance assessments in compliance with quality‑assurance activities.
  • Use the Electronic Health Record (EHR) system proficiently to ensure adequate care of patients and communicate with group providers, partners, and administration.
  • Coordinate follow‑up scheduling with the caseload; new‑patient scheduling and benefits checking are provided by CSRs.
  • Consult with patients and families regarding nutrition needs and goals.
  • Input dietary information accurately and in a timely manner.
  • Exhibit excellent customer service and a positive attitude toward patients.
  • Follow all agency and program protocols to maintain compliance with OSHA regulations and Joint Commission standards.
  • Provide nutrition counseling to program participants.
  • Utilize motivational interviewing techniques during counseling sessions.
  • Develop a referral network of community resources to address patient needs.
  • Be knowledgeable about the impact of nutrition assessment as a preventive measure for chronic diseases.
  • Perform all other duties assigned by the Director of the Chronic Disease Prevention Program in keeping with the program’s philosophy, purpose, goals, and objectives.
  • Abide by infection‑control policies and procedures.
  • Follow policies and procedures as dictated in the Employee Safety Handbook.

Requirements
  • Education: Minimum BS in Nutrition or a related field. A Registered Dietitian credential is required.
  • Experience: Prior experience in nutritional assessment and planning. Two (2) years in a healthcare facility; post‑acute care experience preferred.
  • Other: Registered with the Commission on Dietetic Registration; bilingual in Spanish preferred.
