This position is located at our South Zarzamora clinic in San Antonio. The total compensation package (base pay + bonus) could exceed $300K depending on experience and location. Base pay range: $/yr - $/yr.
Responsibilities- Evaluates and treats center patients in accordance with standards of care.
- Follows level of medical care and quality for patients and monitors care using available data and chart reviews.
- Coordinates patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
- Actively participates as a medical expert with the care team through daily huddles.
- Supports a cooperative practice environment and professional demeanor with the Regional Medical Director and Center Administrator.
- Completes medical record documentation in a timely manner with a quality-based coder to optimize coding specificity.
- Follows policy and protocol defined by Clinical Leadership.
- Meets with the Regional Medical Director about quality of care, outcome data, policy, procedures and records issues.
- Participates in growth opportunities for new or existing services within the Center.
- Participates in the local primary care on-call program as needed.
- Ensures compliance with licensing, certification and accrediting bodies.
- Spends 100% of time clinically focused on direct patient care, including patient-facing time and related administrative tasks.
- Current and unrestricted medical license or willingness to obtain in the state of TX
- Graduate of an accredited MD or DO program
- Board Certification or eligible in Family Medicine, Internal Medicine or Geriatric Medicine (preferred)
- Active and unrestricted DEA license
- Excellent verbal and written communication skills
- Strong interpersonal skills and ability to work within an integrated team-based care model
- Willingness to learn/adapt to a value-based care setting
- Superior patient/customer service
- Basic computer skills, including email and EMR
- TB screening may be required if selected for this role
- Medicare Provider Number
- Medicaid Provider Number
- 2-5 years of directly applicable experience (preferred)
- Experience managing Medicare Advantage panel and knowledge of Medicare guidelines, HEDIS indicators
- Bilingual in English and Spanish
- Guaranteed base salary plus bi-annual bonus
