This position is located at our South Zarzamora clinic in San Antonio. The total compensation package (base pay + bonus) could exceed $300K depending on experience and location. Base pay range: $/yr - $/yr.

Responsibilities
  • Evaluates and treats center patients in accordance with standards of care.
  • Follows level of medical care and quality for patients and monitors care using available data and chart reviews.
  • Coordinates patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment and home health care.
  • Actively participates as a medical expert with the care team through daily huddles.
  • Supports a cooperative practice environment and professional demeanor with the Regional Medical Director and Center Administrator.
  • Completes medical record documentation in a timely manner with a quality-based coder to optimize coding specificity.
  • Follows policy and protocol defined by Clinical Leadership.
  • Meets with the Regional Medical Director about quality of care, outcome data, policy, procedures and records issues.
  • Participates in growth opportunities for new or existing services within the Center.
  • Participates in the local primary care on-call program as needed.
  • Ensures compliance with licensing, certification and accrediting bodies.
  • Spends 100% of time clinically focused on direct patient care, including patient-facing time and related administrative tasks.
Qualifications
  • Current and unrestricted medical license or willingness to obtain in the state of TX
  • Graduate of an accredited MD or DO program
  • Board Certification or eligible in Family Medicine, Internal Medicine or Geriatric Medicine (preferred)
  • Active and unrestricted DEA license
  • Excellent verbal and written communication skills
  • Strong interpersonal skills and ability to work within an integrated team-based care model
  • Willingness to learn/adapt to a value-based care setting
  • Superior patient/customer service
  • Basic computer skills, including email and EMR
  • TB screening may be required if selected for this role
Preferred Qualifications
  • Medicare Provider Number
  • Medicaid Provider Number
  • 2-5 years of directly applicable experience (preferred)
  • Experience managing Medicare Advantage panel and knowledge of Medicare guidelines, HEDIS indicators
  • Bilingual in English and Spanish
