RN Clinical Manager, Home Health

Center Well Home Health is looking for an experienced RN Clinical Manager to coordinate and oversee all direct care patient services provided by clinical personnel.

Pay Range

$77,200 – $106,200 per year

Scheduled Weekly Hours

40 hours per week

Responsibilities
  • Develop, plan, implement, analyze, and organize clinical operations for a specific location.
  • Conduct and delegate the assessment and reassessment of patients, including updating of care plans and interpreting patient needs while adhering to company, physician, and health facility procedures/policies.
  • Manage the assignment of caregivers.
  • Responsible for and oversee the delivery of care to all patients served by the location.
  • Instruct and guide clinicians to promote more effective performance and delivery of quality home care services.
  • Assist clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing patient Plan of Care (POC).
  • Monitor cases to ensure documentation is in compliance with regulatory agencies and requirements of third‑party payers.
  • Coordinate communication between team members, attending physicians, and caregivers to ensure the appropriateness of care and outcome planning.
  • Work in conjunction with the Branch Director and Company Finance Department to establish location’s revenue and budget goals.
  • Participate in sales and marketing initiatives.
  • Supervise all clinical employees assigned to a specific location and provide overall direction, coordination, and evaluation of the location.
  • Handle necessary employee corrective action and discipline issues fairly and objectively.
  • Interview, hire, train, and develop direct care clinicians and coach staff in performance and education programs.
  • Assess staff education needs and conduct regular staff education as needed.
  • Analyze situations, identify problems, and evaluate alternative courses of action using performance improvement principles.
  • Review case manager and clinical staff documentation to ensure appropriate care and documentation requirements.
  • Perform QA/PI activities and work with Utilization Review staff on data tracking for performance review and outcomes of care analysis.
  • Provide direct patient care on an infrequent basis and only in times of emergency.
  • Act as Branch Director in absence.
  • Interpret company standards and policies to ensure caregiver clinical documentation meets internal standards.
  • Maintain relationships with referral and community sources and participate in professional organizations and care‑related programs.
  • Perform other related duties as assigned or requested.
Use Your Skills To Make An Impact
  • Graduate of an accredited School of Nursing.
  • Current state license as a Registered Nurse.
  • Proof of current CPR.
  • Valid driver’s license, auto insurance, and reliable transportation.
  • Two years as a Registered Nurse with at least one‑year of management experience in a home care, hospice, or equivalent environment.
