Physiatry APP (Nurse Practitioner / Physician Assistant)
Iconic Care is seeking a full‑time, independent provider to deliver specialized physiatry care at skilled nursing and rehabilitation facilities.
Day‑to‑Day ResponsibilitiesYou will round at 1–3 facility locations 1–3 days per week, providing focused assessments, treatment plans, and joint injections to optimize therapy participation for 25+ patients per day.
Key Responsibilities- Perform focused assessments and develop individualized treatment plans with primary care teams.
- Provide targeted interventions such as joint injections, Botox, dry needling, and medication management.
- Serve as the sole physiatrist on‑site, operating with high autonomy and collaborative support.
- Coordinate care and refer to other specialties or services as needed.
- Current, unrestricted PA or NP license.
- DEA licensed or willing to obtain DEA licensure.
- Experience in nursing home and/or physiatry is a plus.
- Comfort performing peripheral joint injections (knees, shoulders, hips).
- Strong communication skills and collaborative mindset.
- Familiarity with EHR charting.
- Hands‑on training in facility settings.
- Flexible 5‑day workweek with optional weekends.
- Gain experience in joint injections and rehabilitation care.
- Competitive salary starting at $130,000+/year.
