Job DescriptionAs part of the social services team, this position completes biopsychosocial assessments, develops treatment plans, provides individual/group therapy, and assists in discharge planning.
It works collaboratively with the treatment team, families, community resources, and other healthcare providers.
Job Essentials- Prioritize daily tasks including new admissions and discharging patients along with seeing patients on given case load.
- Participate daily in a multi-disciplinary treatment team meeting.
- Conduct psychosocial assessments, ASAM’s and health disparities forms for new admissions.
- Educate, communicate and coordinate discharge plans with patient’s families and support systems.
- Provide daily individual therapy along with psycho educational groups.
- Complete discharge planning including crisis safety plans, family meetings and discharge summaries.
- Complete documentation in a timely manner according to the hospital’s standards.
- Pay Range CSW/ACMHC (Exempt): $32.30‑$42.80/hr
- Pay Range LCSW/CMHC (Exempt): $36.30‑$46.80/hr

- Education:
Master’s Degree in Social Work or a related clinical mental health field is required. - License: CSW, LCSW, ACMHC, CMHC, AMFT, LMFT with the state of Utah required.
- Additional requirements:
CPR certification; successful completion of training for restraints and seclusions within 90 days of employment.
