Job Summary

Provides preventive screening services, education, and outreach to vulnerable populations. This is an opportunity to contribute to the wellness of local communities.

Responsibilities
  • Conduct health screenings for blood sugar, cholesterol, blood pressure, and body weight (screening training available).
  • Provide health counseling and education to increase knowledge of self-sufficiency.
  • Help clients in identifying their health goals; work closely with patients and wellness coach to help patients determine the best course of action for good health.
  • Identify and help coordinate community events and workshops.
  • Participate in various community events helping community members with diabetes education and health needs.
  • Provide outreach to potential and existing clients in the community and at events.
  • Assist in the coordination of client referrals to housing, food, transportation, and other social services as needed.
  • Work with staff and volunteers to establish trusting relationships with clients while providing support and encouragement.
  • Fill out client electronic intake questionnaire and screening assessment.
  • Make regular follow-up calls and assist patients with follow-up appointments.
  • Minimum Qualifications

    High School diploma, or equivalency (one year of education can be substituted for two years of related work experience). Demonstrated relationship-building and effective communication skills.

    This position is patient-sensitive and must fulfill all associated requirements. We protect our patients, coworkers and community by requiring all patient-sensitive employees to be immunized according to CDC standards and hospital policy. Limited exemptions may be made for documented medical contraindications or religious beliefs that object to vaccinations.

    Care is appropriate to the population served.

    Preferences

    Preferred applicants possess childcare/parenting experience, high school education, or an Associate’s degree. This is a B&C: B&B applicant trailing the certification but non‑certified. Bonus: band 1 special eq. This applicant is a B&A in the same line.
