Overview
Job Summary:
The Intensive Outpatient Clinic (IOC) at the University of Utah Health seeks a Licensed Clinical Social Worker (L.C.S.W.) or Clinical Mental Health Counselor (C.M.H.C.) to provide professional counseling services to individuals with complex medical and psychosocial needs. The IOC operates as an interdisciplinary team of medical and behavioral health providers to improve clinical outcomes and reduce overall cost of care. The IOC serves a Medicaid population under a capitated model with University of Utah Health Plans and does not require billing by the therapist.
The IOC is dedicated to delivering integrated care and supports an on-campus presence at the University of Utah Population Health Center near Rose Park. The role reports to the IOC Medical Director and collaborates with medical and behavioral health teams to address the bio-psycho-social needs of the IOC patient population. The position is categorized as on-site required in certain settings.
ResponsibilitiesJob Specific Responsibilities and Accountabilities:
- Provide regular therapy to patients (e.g., weekly, bi-weekly, monthly) to help achieve psychosocial goals and address sources of adversity, including histories of trauma, using evidence-based and trauma-informed strategies.
- Develop and facilitate groups focused on skill building and fostering community and trust among patients.
- Provide acute mental health support via phone and in-person visits.
- Collaborate on the interdisciplinary team, make sound therapeutic recommendations, and support conflict resolution for individuals, couples, and/or groups.
- Develop and implement individualized treatment approaches to meet the needs of each patient and their families.
- Work closely with all treatment team members, including medical providers, to enhance awareness of social and emotional factors affecting patient health and well-being.
- Apply life-span development principles to support patients in achieving behavioral health goals and improving outcomes.
- Deliver high-quality, tailored care in line with IOC policies, procedures, and University of Utah standards of care.
- Identify and arrange community resources to provide individualized assistance (e.g., post-treatment financial assistance, shelter, transportation).
- Identify external resources, such as community agencies, to help patients resume life in their community and empower them to optimize medical and behavioral health independently.
- Collaborate with medical and behavioral health teams to update IOC policies and procedures and participate in clinic process improvement efforts.
- Oversee and supervise Social Workers, Interns, and Caseworkers within their areas of assignment.
Required
- Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential must be obtained within 30 days of hire.
- One of the following:
- Current Licensed Clinical Social Worker (L.C.S.W.) certificate for clinical practice in the State of Utah.
- Current license to practice as a Certified Social Worker in the State of Utah.
This position is patient-sensitive and requires immunization per CDC standards and hospital policy. Limited exemptions may be available for medical contraindications or religious beliefs that object to vaccinations.
