City Creek Post Acute Care in Salt Lake City, UT is a post‑acute care facility that offers long‑term care, short‑term care, and geriatric services. We provide a stable and collaborative in‑house therapy team dedicated to excellent programs for our geriatric clientele.

• Effectively screen and evaluate patients with cognitive and/or ADL functioning disorders and develop an appropriate plan of care following all regulatory and clinical practice standards.
• Utilize standardized assessments and evidence‑based practice to support clinical interventions.
• Provide rehabilitative, skilled, and medically necessary treatment intervention to patients with cognitive and/or ADL functioning disorders per regulatory and clinical practice requirements.
• Administer procedures such as ADL retraining, therapeutic activities, therapeutic exercises, modalities, and use of supportive and assistive devices.
• Assess patient progress and adjust treatments accordingly in the clinical record.
• Evaluate treatment outcomes for effectiveness, modify services based on evaluation, and make referrals as appropriate.
• Provide consultation and counseling to patients, families, caregivers, and other service providers.
• Effectively communicate with supervisors and other health team members regarding patient progress, barriers, and treatment plans.
• Provide a comprehensive discharge summary of services per regulatory requirements.

• Occupational Therapy license is required.
• Open to occupational therapists at all experience levels. New graduates are welcome.
