• Effectively screen/evaluate patients within short‑term and long‑term rehab and outpatient opportunities
• Utilize standardized assessments and evidence‑based practice to support clinical interventions
• Provide rehabilitative, skilled, and medically necessary treatment intervention in short and long‑term settings
• Administer a wide variety of interventions as part of the plan of care based upon individual patient needs
• Assess patient’s progress and adjust treatments accordingly in the clinical record per regulatory and clinical practice requirements
• Provide consultation and counseling to patients, their families, caregivers, and other service providers related to the physical disorders
• Effectively communicate with the One Clinical Team regarding patient progress and barriers
• Provide comprehensive discharge summary of services per regulatory and clinical requirements

Licensed Occupational Therapist. This position is open to OTs at all experience levels.
