Position Summary
Under the supervision of agency prescribers and the Division Director of Treatment Services, the Medical Assistant/Care Navigator performs routine administrative and clinical support functions, including answering phones, scheduling appointments, facilitating client communication, tracking and managing referrals for medication management or primary care services, and assisting clients in tracking their medication. The role provides support to enhance prescriber appointment utilization, administer injections and collect labs, and connect clients with appropriate services to facilitate access to care and support their behavioral health recovery and overall well‑being.
EssentialDuties
- Act as a support person for the APRNs, facilitating communication between prescribers, clients, and clinic staff.
- Manage the APRNs’ appointment schedule to capacity, eliminating no‑shows by scheduling appointments, making reminder calls, and rescheduling as needed.
- Track clients’ medications and assist in completing patient assistance applications.
- Facilitate medication refills and provide injections or draw labs when necessary.
- Engage and build rapport with clients, assist with referrals to link and connect them with services and resources including primary care, mental health treatment, addiction treatment, recovery support, employment specialists, housing specialists, and other social supports.
- Coordinate care with internal and external providers at admission, discharge, and level‑of‑care changes, including tracking and facilitating referrals to PCPs.
- Screen and monitor clients’ common co‑morbidities while educating consumers and team members on potential side effects of treatments and medications.
- Communicate and coordinate care with other medical professionals such as pharmacy staff, medication representatives, and other treatment providers.
- Demonstrate effective communication on the phone, in writing, and via email, exercising discretion on sensitive and confidential matters.
- Maintain records according to requirements, completing all documentation and billing promptly and in compliance with professional standards.
- Maintain professional boundaries and client confidentiality at all times.
- Consistently attend work on a punctual, regular, and predictable basis.
- Maintain good professional relationships with community figures, referral sources, and other professionals; coordinate and schedule with pharmaceutical representatives as needed.
- Continuously enhance professional skills and knowledge through conferences, workshops, courses, and reading professional materials relevant to job requirements.
- Maintain and adhere to Utah laws regarding confidentiality and mandatory reporting requirements.
- Conduct professional life in accordance with discipline standards and agency clinical policies and procedures.
- Keep abreast of issues and trends in the behavioral health field, including comorbid mental health issues, advances in psychopharmacology, theory, and evidence‑based practices.
- Perform any other functions as necessary or assigned.
- Successful completion of an Accredited Medical Assistant program and a High School diploma or GED equivalent.
- Knowledge of Microsoft Office Suite and multiple phone line systems.
- Ability to communicate effectively with staff and clients.
- Experience in an outpatient mental health setting preferred.
- Demonstrates sound judgment within the scope of responsibilities.
- Maintains professionalism and adapts to change.
- Organized, detail‑oriented, self‑starter.
- Excellent verbal and written communication skills; strong attention to detail.
- Proficiency in crisis intervention.
- Driving required: age 21+, current driver’s license, good driving record, insurable under agency liability policy.
- Must pass Utah‑DHS – Office of Licensing background screening and pre‑employment drug screen.
- Lift and carry up to 25 lbs.
- Walk up and down stairs.
- Move around the facility, interacting with clients for extended periods.
- Work inside and outside an office environment, including climbing stairs, entering client homes, and delivering services in shelters or on the street.
