Job Title: Social Work Assistant Team Manager – Community Led Support Team

Salary: £53,886 per annum

This role is crucial in supporting individuals to live independently by ensuring their needs are assessed in line with the Care Act 2014 and promoting a strengths‑based approach to social work.
The successful candidate will work within a multi‑disciplinary team, carrying out assessments, developing care plans, and managing a caseload of service users while ensuring compliance with legislative frameworks. Key Responsibilities- Carry out Care Act assessments to identify service users’ eligible needs.
- Apply Fair Access to Care Services (FACS) eligibility criteria.
- Ensure assessments are person‑centred, empowering individuals to maximise their independence.
- Develop and implement Support Plans and Care Plans in collaboration with service users and their carers.
- Identify the most effective support to promote independence and prevent escalation of care needs.
- Arrange for services, equipment, and minor adaptations as required.
- Work within an integrated service model, collaborating with social care, health, education professionals, and community resources.
- Signpost individuals to appropriate external agencies and services.
- Participate in multi‑disciplinary meetings to ensure a holistic approach to care planning.
- Ensure compliance with the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.
- Monitor risk and elevate safeguarding concerns in line with procedures.
- Support service users in making informed decisions while balancing risk and independence.
- Review Support Plans and Care Plans to monitor service delivery effectiveness.
- Ensure that outcomes and objectives are being met, adjusting support where necessary.
- Collect and share relevant data and information with the Team Manager to improve service provision.
- Degree in Social Work (or equivalent) from a UK university
- Registration with Social Work England
- Post‑qualification training in mental health or related areas (e.g., Mental Health Act, Mental Capacity Act, Safeguarding)
- Significant post‑qualification experience, including substantial experience in adult social work or mental health settings.
- Experience of working in a leadership or management role, including supervising and supporting staff.
- Experience of managing complex cases and working with individuals with mental health conditions, including risk assessment and safeguarding.
- Strong knowledge of mental health legislation, including the Mental Health Act, Care Act, Mental Capacity Act, and safeguarding procedures.
- Excellent communication, interpersonal, and organisational skills.
- Ability to lead and manage a team effectively, ensuring high standards of social work practice and service delivery.
- Experience of multi‑agency working and promoting collaborative service delivery.
