• Provide compassionate social work services to clients within a healthcare setting, focusing on geriatric care where applicable.
• Collaborate with the healthcare team to coordinate care, advocate for patients, and support treatment plans.
• Maintain documentation, ensure compliance with applicable standards, and participate in continuous quality improvement.

• High School diploma required.
• Bachelor’s degree in Social Work or a Human Services field desirable.
• Two years of social work supervised experience working directly with geriatric clients in a health care setting desirable.
